Behavioral Health Facilitator - PRN (As Needed)

Position: Behavioral Health Facilitator Location: Position could be based out of Chariton or Corydon. Will require travel to all other locations as needed for coverage. Summary: Assist in coordinating communication between psychiatric providers ensuring consistent patient care and coordination of services. Experience: Qualified applicants must be well organized and computer literate. Education: High school diploma or GED. Preference will be given to candidates with CNA, CMA, or LPN license. Prior completion of Medication Manager class is preferred.
