Working under the supervision of a Registered Nurse or licensed provider, the LPN assists in delivering high-quality, patient-centered care to diverse and underserved populations.Requirements:Competencies/Responsibilities:*Include but are not limited to:Prepare patients for examination by obtaining vital signs, medical history updates, and medication reconciliationAssist providers during patient examinations and proceduresAdminister medications, injections, and immunizations according to provider orders and standing protocolsPerform point-of-care testing and collect laboratory specimens as appropriateSupport daily clinic operations and patient flowMaintain accurate and timely documentation in the electronic health recordAssist with care coordination activities including referrals, lab tracking, and follow-up careSupport preventive screening initiatives including immunizations and health screeningsAssist providers with chronic disease management activities such as diabetes, hypertension, and asthma monitoringParticipate in outreach and follow-up efforts to improve patient adherence to care plansWork collaboratively with providers, registered nurses, medical assistants, and care coordination staffParticipate in daily team huddles and care planning activitiesCommunicate patient concerns or changes in condition to appropriate clinical staffAdhere to clinical policies, procedures, and infection control guidelinesSupport organizational quality improvement initiatives and reporting requirementsMaintain patient confidentiality and comply with HIPAA regulationsKnowledge of basic nursing principles and clinical proceduresAbility to work effectively in a fast-paced outpatient environmentStrong interpersonal and communication skillsAbility to work collaboratively in a team-based care modelCommitment to providing culturally competent care to diverse populationsMinimum Education and Experience:Required:Graduate of an accredited Practical Nursing programCurrent Licensed Practical Nurse license in the State of OhioBasic Life Support (BLS) certification requiredOne year of clinical experience preferredExperience in primary care, community health, or ambulatory care preferredExperience with electronic health records preferredPhysical Requirements:Ability to stand, walk, and move throughout the clinic during shiftsAbility to lift and assist patients as neededManual dexterity required for clinical procedures and equipment operationCompensation details: 27-30 Hourly WagePIdd37b35fe9a7-26289-39952494
