Job Description Our organization is in search of a dedicated and empathetic Physical Therapist (PT) to become a part of our Healthcare & Medical Services team, providing support to residents within our senior living community. In this position, you will play a crucial role in assisting older adults in maintaining or regaining their independence, mobility, and overall quality of life in a nurturing and supportive atmosphere.Key ResponsibilitiesConduct thorough evaluations of residents' functional capabilities, assessing strength, range of motion, balance, gait, and endurance.Utilize your clinical knowledge to create personalized, evidence-based treatment plans focused on enhancing functional mobility, alleviating pain, preventing falls, and increasing engagement in daily activities.Implement therapeutic exercises, gait and balance training, neuromuscular re-education, and manual techniques as needed, working individually with residents to help them achieve their goals.Collaborate closely with nursing staff, physicians, and the interdisciplinary care team to ensure a cohesive approach to resident care.Engage in regular communication and participate in care conferences to advocate for residents, modify treatment plans as necessary, and provide clinical insights that uphold the highest standards of care.Educate residents, families, and caregivers on safe mobility practices, the use of assistive devices, home exercise routines, and strategies for fall prevention to encourage ongoing success beyond therapy sessions.Maintain accurate and timely documentation of assessments, treatment sessions, progress notes, and care plans, ensuring compliance with state and federal regulations as well as professional standards.Support our commitment to high-quality, person-centered care by keeping detailed records that reflect each resident's progress and response to therapy.Contribute to wellness and restorative programs within the community by participating in group activities, balance and strengthening classes, and initiatives aimed at keeping residents active and engaged.QualificationsMust be a licensed Physical Therapist with a passion for geriatric care.Excellent communication skills and a resident-first mindset are essential.Ability to work collaboratively within a mission-driven team.Working ConditionsThis role provides a unique opportunity to make a significant impact daily, utilizing your professional expertise to enhance residents' physical well-being while promoting dignity, independence, and personal growth.Benefits and Growth OpportunitiesWe offer a supportive environment that encourages professional development and growth, allowing you to thrive in your career while making a difference in the lives of our residents.If you are enthusiastic about joining a collaborative team dedicated to improving the lives of older adults, we encourage you to explore this opportunity with us.
