Join us in creating a better way to care.$500 Sign-On Bonus!OverviewThe Medical Assistant - Clinical Support Coordinator provides direct patient contact in support of the Primary Care Provider to complete patient triage, and medication reconciliation, and provide orders to Assisted Living facility staff as director by the Primary Care Provider. This role facilitates communication and patient care as the main liaison between the assisted living staff and the primary care provider.Duties & ResponsibilitiesDemonstrates kindness and compassion in all patient interactionsCoordinates care with providers for patients residing in assisted living facilitiesWork with our Assisted living partner staff to coordinate patient care needsCollects and documents vital signs, histories, and screeningsProvides instruction to the patient regarding medications and dietAuthorizes prescription refills as directedFacilitates the completion of needed documentation with patients/POAsCoordinates the distribution of remote patient monitoring (RPM) equipmentTracks and reports RPM results to providersCompletes medication reconciliations for assisted living facilitiesPerforms phlebotomy and correctly processes specimensCleans and maintains treatment rooms and equipmentCoordinates follow-up appointments with patientsPerforms quality controls and equipment checks.QualificationsCertified Medical Assistant1 year of experience preferredCurrent BLS certification for healthcare providers preferredHigh School Diploma/Equivalency requiredExperience in primary care, family practice, internal medicine, urgent care or ER preferredThe working environment and physical requirements of the job include:Work is performed indoors in an Assisted Living Community setting with air conditioning, artificial light, and an open workspace.In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculators, copiers, and FAX machines. Good reasoning ability is important.
