RNRolling Meadows of Strum Assisted Living & Memory Care is accepting applications for an RN. Position Overview The RN is responsible for regulatory and state compliance, documentation, resident care, medication administration, staff training and shared oversight of all local management for communities. Job Duties Knowledge and understanding of all Nursing responsibilities as outlined in the State DHS 83 guidelines Maintain a valid RN License Manage and oversee medication delivery as assigned Review Service Plans as assigned for residents who have specific medical issues, such as diabetics, residents receiving blood thinners, etc. Participate and develop resident ISP's, ensure they are updated with any resident change in condition or every 6 months and oversee their compliance Oversee and ensure that all resident medication programs are carried out and adhered to and properly logged in electronic documentation software and filed accordingly Train and audit to maintain DHS compliance Review and ensure that Monthly and Quarterly psychotropic reviews are completed MAR Audits - Monitor/scheduled audits/reviews, random audits/reviews of medication carts, medication administration, medication rooms, medication renewals and medication delivery processes. Oversee Cycle Fill - monitor for completion and follow up for missing meds Oversee new medication orders in MAR, Faxes for compliance, discontinued meds Review of MAR sheets and document summary notes on a routine basis for each resident Verify MD orders when applicable Working with MD's, Pharmacy and other 3rd party vendors Work with pharmacy provider and will conduct annual/periodic audits of the medication delivery system Random audits of charts, care plans & ISP's, medication orders Assist in performing prospective resident assessments Provide clinical input for resident ISP- Individual Service Plans Assist with ISP meetings with appropriate teams; if needed based on change in condition Provide input for resident care plan meetings if needed or required Review and make recommendations on the 30-day, 6 month and yearly assessments Consult with physicians as necessary in providing care, treatment, diagnostic and therapeutic services Assist when applicable to report to the state of Wisconsin Oversight and RN delegation of medication administration duties for state certified CBRF Caregivers; Requalify medication delegation due to medication error, medication training Ability to effectively train and in-service relevant skilled cares including diabetic injections, suppositories and breathing treatments to certified care providers; Communicate any medication and/or treatment changes to care providers Skin reviews Work with operations, community administrator teams, dietary and care staff to ensure quality of care Provide staff education and mentoring as assigned Provide new hire health screens Work with caregivers to regularly assess skills and make recommendations for training opportunities Other duties as assigned by the Administration. Basic understanding of infection control as it pertains to COVID 19 and other pandemic policies Participate in monthly QA Committee is to review and identify the root cause of the aforementioned items and to reduce their reoccurrence.
