Physical Therapist, Home Health Position Responsibilities: A qualified professional, who directs, supervises, evaluates and provides physical therapy services to adult patients in the home, following physician’s orders. Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures. Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment. Recommend modification of plan of care to physician, as needed. Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date. Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence. Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.). Include and educate the patient and caregiver(s) regarding treatment plan and goals. Attend physical therapy staff meetings and in-service presentations. Participate in case conferences. Required Skills/Qualifications: Graduate of an accredited Physical Therapy program. Current, valid Physical Therapy license issued by NYS Department of Education. One year of practical experience as a licensed physical therapist.
