Home Health: Speech Language Pathologist/Speech Therapist

Description The Speech Language Pathologist/Speech Therapist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.Position: Speech Language Pathologist/Speech Therapistfor Home HealthHourly + PPV: $45 hourly, depending on experience plus Pay-Per-Visit: Therapy SOC $145. Therapy Eval, Recert, Oasis Discharge, Reassess $85 and Therapy Visit $70Service Area: Southeast Metro (coverage area: Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul Park, Rosemount, Eagan, Woodbury & surrounding). Plus, mileage reimbursement of 72.5 cents per mile.Schedule: Casual/Part-Time. That's where our people come in - from accounting and health technology to front-line nurses, advanced practice professionals, caregivers and everything in between, we are all invested entrepreneurs focused on helping people age magnificently.Position Functions & Responsibilities:Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.Identifies functional speech defects and establishes a plan of care to improve client's function.Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.Recommends communication devices/aids as indicated.Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.Prepares and submits notes per organization policy.Maintains clinical competency in speech-language pathology practice and theory.Identifies client and family/caregiver needs for other services and refers as appropriate.Instructs client, family/caregiver and other organization health care personnel in the client's treatment regime as indicated.May supervise home health aide in procedures directly related to the speech-language therapy plan of care.Performs other duties as required to facilitate the delivery of speech-language pathology services.Lead by example in establishing rapport with clients, clinicians, partners, and the community.Ensure positive/professional communication within the teamCustomer service resolution; escalate concernsPrepares clinical notes and updates the primary physician when necessary and at least every 60 days.Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required.Communicates with community health related persons to coordinate the care plan.Timely completion of all documentation as it relates to the care plan by 10am the next dayUnderstands and complies with all the rules and regulations that govern the responsibilities of a registered nurseCompletes orientation and annual training requirements as assignedParticipates in Lifespark's Event Management process including initiating reports, escalation of events and investigationsPromotes safety in the workplace through real time coaching and participation in safety awareness trainingInstructs, supervises, and evaluates home health aide care provided every two (2) weeksManages client supplies assuring appropriate DME provider is utilized.Schedules and keeps member appointmentsOrganizes travel efficiently and documents mileage accurately within EMROther duties as assignedQualifications:Minimum of one year of clinical experience in Speech Language PathologyPossesses a master's or doctoral degree in speech language pathologyCurrent Minnesota Speech Language Pathologist LicenseContact Lifespark Now!Brooke Peiffer, Talent Acquisition SpecialistEmail: bpeiffer@lifespark.comPhone: 248-906-8292Book a time to Chat: https://calendly.com/bpeiffer/phone-interview-1Our Lifespark culture has created not only an award-winning workplace - earning Star Tribune Top Workplace 12 times, Minneapolis Business Journal Best Places to Work three times, and Top USA #1 in Healthcare - but a place where you have the room to be creative, make a difference, and have a purposeful, direct impact on how people age.
