This unique opportunity places you at the heart of a School-Based Health Center, where you'll provide vital care to students and help shape healthier futures every day.Whether you're looking for a 10, 11, or 12-month schedule, this role offers flexibility, purpose, and the chance to build meaningful relationships with the community you serve.As a Nurse Practitioner at ConnextCare, you will:Provide comprehensive patient care, including diagnosis, treatment, and referralsDeliver care within your scope of practice, collaborating with physicians as neededMaintain accurate, compliant medical records in accordance with all regulationsParticipate in quality improvement initiatives, peer review, and continuing educationSupport development and monitoring of healthcare goals and quality metricsEngage in community outreach, helping to promote the role of the Nurse PractitionerCollaborate with leadership and medical staff to improve care delivery and efficiencyContribute to Patient-Centered Medical Home (PCMH) initiativesRequirementsKnowledge/ Education Requirements:A. Valid/active NYS RN and NP licensureB. Masters degree in a NursingC. Experience in family practice preferred, but not requiredD. Computer proficiency in electronic health records, health information technology and health informatics requiredE. Promote the organization's code of conduct, safety rules, and adherence to all company policiesF. Carry out job responsibilities in an ethical, effective, comprehensive and professional mannerG.
