Assistant Manager, Care Coordination
DescriptionShodair is looking for an Assistant Manager, Care Coordination to provide operational and strategic leadership for care coordination across inpatient, outpatient, and financial services. This role oversees multidisciplinary teams, improves care transitions, and drives outcomes related to quality, patient experience, and financial performance.Key ResponsibilitiesLead, coach, and develop care coordination teams; oversee hiring and performance management Oversee care planning, discharge processes, and complex case escalations Ensure coordination across clinical teams, families, and community partners Monitor workflows, KPIs, and departmental performance (access, throughput, outcomes) Drive process improvement, documentation integrity, and reimbursement optimization Ensure compliance with HIPAA, CMS, and accreditation standards Partner with leadership, finance, and patient access on strategic initiatives Build and maintain external partnerships to support care transitionsWhy This Role MattersThis position plays a direct role in improving patient outcomes, strengthening care transitions, and ensuring efficient, high-quality care delivery across the organization.
