Tailored Care Manager Supervisor - Charlotte OfficeEpiphany Family Services, LLCCharlotte, NCHybrid Position (In-Office, Field Work, and Work from Home)Internal Candidates, not eligible for bonus.About the RoleEpiphany Family Services is seeking an experienced Tailored Care Manager Supervisor to lead a team of Care Managers providing whole-person, integrated care to individuals with complex behavioral health and medical needs.This position supports individuals with Intellectual/Developmental Disabilities (I/DD), Traumatic Brain Injury (TBI), serious mental illness (SMI), and substance use disorders (SUD).The supervisor will ensure high-quality care management services, regulatory compliance, and strong collaboration with healthcare providers, community partners, and care teams.This role operates in a hybrid work environment, including office collaboration, community field visits, and remote work from home.ResponsibilitiesLeadership & SupervisionSupervise, mentor, and support a team of Tailored Care ManagersProvide coaching, supervision, and performance managementEnsure staff meet productivity and documentation standardsCare Management OversightOversee whole-person care planning and coordinationMonitor caseload assignments and service deliveryEnsure care gaps are addressed and members receive appropriate servicesCompliance & QualityEnsure compliance with NC Medicaid Tailored Plan requirementsConduct case record reviews and quality assurance monitoringMaintain compliance with 10A NCAC 27G standardsCollaborationWork closely with healthcare providers, hospitals, and community partnersSupport integrated care teams to improve member outcomesIdentify service gaps and improve service deliveryRequired QualificationsCandidates must meet Qualified Professional (QP) requirements under 10A NCAC 27G regulationsEducation & Credential RequirementsApplicants must meet one of the following:Licensed or provisionally licensed clinician (LCSW, LCMHC, LCAS, RN, etc.)Registered Nurse licensed in North Carolina with 4 years MH/DD/SUD experienceMaster’s degree in a human services field + 1-year supervised MH/DD/SUD experienceBachelor’s degree in the human services field + 2 years’ supervised MH/DD/SUD experienceBachelor’s degree in another field + 4 years’ supervised MH/DD/SUD experienceFully certified NCASPPB professional with 4 years MH/DD/SUD experienceNote: CADC-R and CADC-I certifications alone do not meet QP requirements without the required degree and experience.Experience RequirementsMinimum 2+ years experience in Tailored Care Management (TCM)Minimum 3 years supervisory or leadership experienceExperience working with individuals with:I/DDTBISerious Mental IllnessSubstance Use DisordersPreferred QualificationsExperience working within NC Medicaid Tailored PlansExperience with Electronic Health Records (EHR)Knowledge of DSM-5Experience supervising behavioral health or care management teamsBenefitsEpiphany Family Services offers a competitive benefits package including:Medical InsuranceDental InsuranceVision InsuranceShort-Term Disability (STD)Long-Term Disability (LTD)80 Hours Paid Time Off (PTO)Paid HolidaysEmployee Discount Program through Tickets at WorkWhy Join Epiphany Family Services?At Epiphany Family Services, we are committed to improving lives through person-centered, integrated care.
