Pharmacist Location: Panama City, FL Agency: Mindlance Health Pay: $2,207 per week Shift Information: Nights - 4 days x 10 hours Contract Duration: 26 Weeks Start Date: ASAP About the Position ID: 76397803 Shift: 20:00-06:30 Description: - TRAVEL - PHARMACIST- COST CENTER (712) /8:00PM-6:30AMMUST HAVE HOSPITAL EXPERIENCE7 on 7 off Unit Profile The clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Must have: PharmD or Bachelor s degree with recent acute care hospital experience, current pharmacist licensure in Florida Advanced skills needed: Ability to accurately check and/or double check pediatric oral and intravenous solutions Experience in Pharmaceutical Calculations and PK dosing of Aminoglycoside and Vancomycin Experience in TPN calculations Ability to solve issues with Pyxis Procedures performed on unit: Practices and adheres to the Code of Conduct and Mission and Value Statement. Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervenes when orders are unclear or potentially unsafe or inappropriate Interprets medication orders (verbal and written), transcribes to computerized patient medication profiles accurately, follows facility practices and procedures for documentation in the patient s medical record and trouble shoots/clarifies Centralized Order Entry (COE) orders as appropriate. Prepares and dispenses medications, including sterile, chemotherapy (limited), and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines. Based on knowledge and expertise, develops, assists, implements, and executes all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy-directed dosing Issues controlled substances to patient care areas and maintains records as required by law and follows corporate policies and procedures related to the dispensing and monitoring of controlled substances Reads, extracts, and interprets information in patient charts accurately Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner, shares good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services and reviews medication related events that have occurred to prevent recurrences Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians.
