This role involves inpatient case management with a strong focus on timely discharge planning. The successful candidate will work day shifts from 8:00 AM to 4:30 PM, with a commitment to every other weekend, totaling a 40-hour work week.Key ResponsibilitiesConduct comprehensive assessments of patients' needs and care requirements.Collaborate with interdisciplinary teams to develop and implement effective discharge plans.Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.Educate patients and families on discharge processes and post-discharge care.Maintain accurate documentation of assessments, interventions, and patient progress.Adhere to hospital policies and regulatory requirements regarding patient care.QualificationsExperience: Previous inpatient RN case management experience in an acute hospital setting is required.Education: Current Registered Nurse (RN) license in the state of California.Certification: Case Management certification (CCM or ACM) preferred but not required.Skills:Strong critical thinking and problem-solving abilities.Excellent communication and interpersonal skills.Ability to work collaboratively in a fast-paced environment.
