Location: Riverside County South About This Opportunity The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician. Qualifications Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent. Licensed or registered to practice as a PT in the state of practice. Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician's health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute. Has the ability to read and follow written instructions and document care given. Is self-directing and has the ability to work with little direct supervision. Possesses and maintains current CPR certification. Is flexible and cooperative in fulfilling the role obligations. Demonstrates tact, patience, and good personal hygiene. Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements. Must be able to verbally communicate in the language of the client and document all client-related activity practices. Responsibilities Providing treatment as ordered by the attending physician, dentist, or podiatrist. Assisting in developing and updating the plan of treatment or Plan of Care (POC). Assisting the physician in evaluating level of function. Observing, recording, and reporting information on the patient's condition to the attending physician, case manager, and in the patient's health record. Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient's therapy program. Teaching and supervising other healthcare personnel when appropriate. Evaluating the home environment and making appropriate recommendations. Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures. Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Why Join us? Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
