WIC Nutrition Aide III (Call-In)
Primary Purpose: Under indirect supervision, accurately and efficiently certify/recertify WIC participants based upon Federal regulations and State of Hawaii Department of Health WIC policies using the HIWIC computer system. Assists in all aspects of WIC services. Supports the mission, goals and objectives of Kalihi-Palama Health CenterEssential Duties:Maintain professional customer service towards all participants and staffPerforms routine clerical duties with ability to type 25wpm, use fax/scan/copy machineAccurately orients participants to the WIC programGive directions to KPHC WIC clinic via all modes of transportationAble to use audio visual equipment for WIC orientation videoAccurately explains the WIC program/history/foods approvedAble to accurately and efficiently certify/recertify WIC participants based upon Federal regulations, and State of Hawaii Department of Health WIC policiesComplete presumptive eligibility whenever possible and schedule follow up accordinglyCan accurately, verbally outline and record all aspects of WIC eligibility requirements, ID, household income eligibility and what documents are required to prove eligibility and Med-Quest verificationUse the HIWIC system to perform essential duties and other duties as assignedAccurately determines and records first and second authorized caregiversAccurately able to verbally explain e-WIC cardAccurately able to issue and activate e-WIC cardAccurately able to assist setting participants PINAble to direct participants in resetting/unlocking e-WIC pin using IVR numberAbility to deactivate card, set appointment for participants to retrieve new cardAble to retrieve and assess necessary foster care documents and legal guardianship documentsAbility to follow policies and procedures to issue e-WIC card and benefits to foster clientsAccurately records new pregnancy and end of pregnancyAccurately complete in-state WIC transfersAccurately print Verification of Certification (VOC)Knows contact information to other WIC programs to help clients transfer if neededAccurately apply for Hawaii WIC, issues e-WIC card and benefits to out of state transfersChecks participants address/zip and phone number at each visitAnswer the phone, take messages and return phone calls in a timely mannerRetrieve phone messages/emails and returns calls/email in a timely mannerAccurately measures, records, and interprets anthropometric dataAccurately measures (Masimo), records and interprets hematological dataMust be effective in motivational interviewingMust be effective in setting and documenting SMART goals with WIC clientsAccurately creates standard and tailored food packagesAccurately creates Special Authorized food packages with approved authorityIssue e-WIC benefits/explain usageCan correctly verbally discuss all WIC eligible foods/formulaAble to void and re-issue-WIC cards/WIC benefitsAble to modify benefit issuance/retailor foodsCorrectly does nutrition education (NE) contacts for all low-risk clients, read medical notes, gets needed forms and makes correct next appointmentCorrectly provides appropriate follow-up nutrition education counseling to high-risk clients after seeing the registered dietitian at least once per certificationFollow "Special Formula and Medical Nutritional Products Issuance Guide" to complete special authorization in HIWIC with approved authorityAble to follow Special Formula ordering, receiving and issuing guidelinesAble to look up history of contacts to determine if cards were signed electronicallyAble to do monthly time study for State Department of Health WIC officeAble to provide nutrition education for pregnant women, breastfeeding women and postpartum womenAble to provide nutrition education for infants and children (i.e. breastfeeding, bottle use, low iron, overweight, underweight, etc.)Able to retrieve nutrition education from WICHealth.org and accurately record online nutrition education in HIWIC.Able to assess WIC clients and appropriately refer high risk clients to the WIC Registered DietitianCall WIC "No Show" list monthly to contact missed appointmentsSend WIC letters for applying to WIC, WIC orientation, and missed appointmentsRetrieve WIC referrals in Centricity (KPHC EMR)Schedule and complete WIC referrals in HIWIC in a timely manner according to standardsAppropriately refer WIC clients to Kalihi-Palama Health Center for additional resourcesAppropriately refer to other community resources/healthcare providers.Assist with outreach activitiesDocument and assist with voter registrationDocument immunization statusScan all necessary and required documents in HIWICAccurately documents WIC Rights and Responsibilities signature(s) for primary and alternate caregiversAble to follow ethical guidelines and assure participant confidentiality on all information (HIPAA).Able to provide interpretation in proficient language(s)Regular and reliable on-site attendance is required.Other Duties:Observe safety and security procedures;
