OverviewThe WOCN provides expert care and coordination for patients requiring wound, ostomy, and continence management in the home setting. This role supports high-quality patient outcomes through advanced clinical practice, staff education, and collaboration with the interdisciplinary care team.This position will cover Western Nassau and Queens.5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating WeekendsGreat Healthcare benefits, and education reimbursement of $5,250 annuallyAdditional compensation for visits over your caseloadJob DetailsKey ResponsibilitiesAssess, plan, implement, and evaluate patient care in the home settingProvide specialized wound, ostomy, and continence treatmentsMonitor patient progress and adjust care plans as neededServe as a clinical resource and mentor to nursing staffSupervise Home Health Aides and support care team performanceEnsure accurate documentation and compliance with regulatory standardsCollaborate with physicians and healthcare providers to ensure continuity of careParticipate in quality improvement and patient outcome initiativesQualificationsBachelor's Degree in Nursing (BSN) requiredCurrent New York State Registered Nurse (RN) licenseMinimum 5 years of community health nursing experienceStrong clinical expertise in wound, ostomy, and continence careCWOCN (Certified Wound, Ostomy, and Continence Nurse) requiredExcellent communication, organizational, and problem-solving skillsSalary RangeUSD $105,000.00 - USD $136,500.00 /Yr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
