Position Summary
We are seeking a Physician (MD) to provide mobile primary care and rounding services across senior living communities. In this role, you will manage a panel of patients, deliver comprehensive care, and serve as a key clinical leader within each community. This is an opportunity to practice at the top of your license, influence outcomes, and build long-term relationships with patients and care teams in a growing, high-demand model.
Responsibilities
Provide on-site rounding and primary care services in assisted living, memory care, and long-term care settings.
Perform diagnostics, treatment planning, and chronic disease management.
Reduce ER visits and hospital readmissions through proactive, consistent care.
Collaborate with advanced practice providers, nursing staff, and interdisciplinary teams.
Act as a clinical leader and resource within each community.
Communicate with families, caregivers, and facility leadership.
Ensure accurate documentation, EMR utilization, and follow-up care.
Qualifications
Active Minnesota Physician (MD) license in good standing.
Experience in geriatrics, internal medicine, family medicine, or post-acute care preferred.
Strong ability to manage complex patient populations and make independent clinical decisions.
Valid driver’s license and reliable transportation.
Commitment to high‑quality, patient‑centered senior care.
