About the Role: Are you passionate about making a real difference in the lives of families in need? As a Care Coordinator (Wraparound Facilitator), you'll be at the heart of our mission, helping adolescents (ages 5-21) and their families navigate challenges and build stronger, healthier futures. Your role will involve building and working closely with a team to create personalized support plans and coordinate a range of services that make a meaningful impact. What You'll Do: Connect with Families and Team Members: Reach out to new families within 3 days and meet with them face-to-face within 7 days to start building strong relationships. Lead the Way: Build a team of supports and facilitate monthly Child and Family Team Meetings (CFTMs), keeping everyone updated with an evolving Plan of Care. Create and Customize: Develop and monitor personalized Plans of Care, ensuring they meet each family's unique needs and goals. Coordinate with Care: Make sure families get the educational, social, and mental health support they need, and ensure these services are high-quality and accessible. Keep It All on Track: Maintain detailed and timely records, including updates in our Electronic Health Record System. Safeguard Privacy: Handle all information with the utmost confidentiality, adhering to HIPAA, FERPA, and other privacy standards. Who We're Looking For: Education & Experience: You have a bachelor's degree in a human services field (or a related field with at least 2 years of experience in family, children/youth, or community services). Training: You're ready to complete the required training and credentialing for Wraparound care coordination. Skills & Qualities: Strong communicator, organized, and great with people. Your enthusiasm for helping families shines through in everything you do. Practical Needs: Reliable transportation, a valid driver's license, and auto insurance are essential. Why Join Us?
