Independent Living Program Manager
THIS POSIITON MAY BE LOCATED AT ANY FACILITY WITHIN NWFHN'S CIRCUIT 2 or CIRCUIT 14 SERVICE AREA.
POSITION OBJECTIVE: To direct, manage and develop a group of independent living specialists within the service area. To develop and build a program which promotes the well-being and independence of youth and young adults involved in the dependency system. Independence should be promoted by supporting the youth and young adult’s educational success, development of employment skills, establishing a network of supportive adults, along with an overall focus on well-being.
ESSENTIAL FUNCTIONS:
· Implement, coordinate and supervise a group of independent living specialists who serve as primary case manager for young adults age 18 -23 who are active Extended Foster Care, Postsecondary Education Services and Support (PESS) or Aftercare Services
· Monitor and verify eligibility for Extended Foster Care, PESS and Aftercare Services
· Ensure all case activity is documented in the Florida Safe Family Network (FSFN) and ensure case file contains appropriate eligibility documentation
· Ensure NWFHN and the Independent Living Program are present and represented well in all court systems
· Ensure support is provided to CMO’s in providing life skills to youth in the dependency court system as well as monitor completion and quality of transition plans being developed for 16 and 17 year olds
· Monitor, manage and develop improvement plan as necessary for NWFHN’s Key Performance Indicators and contract measures.
· Assist contract and grant renewal process for independent living programs.
· Establish and maintain positive collaborative relationships with relevant community based care agencies.
· Establish and maintain community partnerships with educational institutions and community businesses to facilitate client development
· Develop and implement new practices that improve the quality of service to clients.
· Participate in fund-raising and grant writing activities as needed.
· Monitor and manage overtime expenditures.
· Participate in community based care partnership meetings as a representative of the agency.
· Perform supervisory responsibilities specified in COA Standard TS 3:
· Delegate and oversee work assignment so that service delivery is performed according to the organization's mission, policies and procedures and service philosophy
· Provide case consultation and in-service training, as appropriate
· Identify unmet training needs
· Conduct performance evaluations
· Support and enhance staff's ability to perform their jobs by teaching and modeling technical knowledge and skills, work management and communication skills and conflict management skills
· Supervise direct service personnel and perform responsibilities specified in COA Standard TS 3.08:
· Track and monitor the progress of families and individuals receiving services
· Collect and apply data to improve client outcomes,
· Meet the organization's quality improvement and evaluation requirements.
· Participate in the quality improvement process.
QUALIFICATIONS:
:
· Bachelor’s degree in a human services related field (All degrees must be from an accredited college or university.)
· Minimum of four years of supervisory experience in child welfare
· Must be able to work flexible hours
· Valid Driver’s License within 30 days from hire
· Master's degree in a human services related field
· Program management experience
