Patient Care Coordinator

Patient Care CoordinatorThe Patient Care Coordinator will provide customer service and be the first point of contact to patients and Physicians by reviewing patient profiles, scheduling deliveries of patient's medication and scheduling patient nursing services; collecting key clinical information vis URAC patient assessments. Schedule Details Location: On-Site (Laguna Hills, CA) Hours: MondayFriday, 8:30AM - 5:00PM (Hours may vary depending on business needs) Duties and Responsibilities Collect basic patient information as instructed. Help schedule medication deliveries and refills. Update and maintain delivery schedules. Answer patient phone calls and direct questions to the appropriate team member. Provide basic information about medication deliveries and supplies. Help collect required forms and signatures. Assist with entering medication and supply orders under supervision. Communicate with pharmacy and nursing teams to support timely deliveries. Report patient concerns or issues to a supervisor or clinical team members. Required Qualifications High school diploma or equivalent. Previous customer service or administrative experience in a healthcare, pharmacy, or medical office setting. Ability to follow instructions and work as part of a team. Ability to follow company policies and safety guidelines. Experienced with scheduling software and electronic medical records (EMR) systems. Typing speed of at least 45 wpm. Preferred Qualifications Familiarity with basic medical terminology. Experience answering phones and working with patients or customers. Ability to manage multiple tasks in a fast-paced environment.
