The primary goal is to empower clients to reach and maintain their highest level of physical and mental well-being by providing participant-centered care and coordination with local agencies. You will act as a primary advocate, helping individuals navigate the realms of recovery, housing, and employment.Key ResponsibilitiesDirect Engagement: Conduct regular, weekly meetings with program participants to monitor progress and provide guidance.Care Planning: Assess individual needs and develop a participant-centered care plan focused on client-driven goals.Inter-Agency Collaboration: Partner with local agencies and care teams to ensure success in critical areas, including recovery, housing, employment, and reducing recidivism.Documentation: Maintain timely and accurate documentation of each client interaction to ensure high-quality care and compliance.Required QualificationsEducation/Experience: A Bachelor's degree in a field closely related to the position's responsibilities OR four (4) years of experience providing direct services in a human service setting.Licensure : Possession of a valid Driver’s License.Vetting: Ability to successfully complete a background check.Training: Commitment to complete the care coordination training provided by the State of North Dakota after being hired.Preferred QualificationsCase management experience.Excellent writing skills.Demonstrated ability to be a self-starter.Work experience in case management, parole and probation, or addiction services.Compensation And BenefitsJob Type: Part-timePay: Compensation is based on outcomes for each participant.
