BHC provides brief behavioral healthinterventions for patients with chronic illness who\'s emotional and/orbehavioral issues negatively impact thier disease management. The BHC\'srole is to support the ongoing behavioral health interventions of theprimary care provider and the focus is to resolve problems within theprimary care service context. The referring primary care provider, atall times, remains the overall care manager.This position will provide screening, triage, case management, andclinical intervention services to individuals who are receiving care inthe primary or management of behavioral concerns in primary care andassist in the triaging and managing of emergent referrals.**Essential Job Functions:** Service as a core member of the health care team within primarycare. Primary job function is to provide brief clinical intervention to acaseload of primary or specialty care patients per establishedstandards and guidelines. Functions include engagement andrelationship building, assessment, treatment planning, therapeuticinterventions, interdisciplinary coordination, and monitoringreferrals. Secondary function is to assist with resource locationand advocating for benefits. Provide brief intervention therapy for clients who are not in needof longer term mental health services or who present in primary careenvironment who require more immediate attention. Interventionsinclude motivational enhancement counseling, adjustment to illness,treatment adherence issues, behavior modifications stress managementand psycho-education. Provide medical health triage for clients referred by otherproviders in the primary care setting and provide crisisintervention for clients with more emergent behavioral health needs. Effectively collaborate with health care providers and case managerswithin Clearwater Valley and/or St. Mary\'s and the larger communityto meet the needs of mutual clients including coordinatinginterdisciplinary case conferences. Assist patients with transportation issues that impact treatmentadherence. Appropriately refer clients to community resources andconsistently monitor the outcomes of said referrals. Complete documentation of services in a legible manner in accordancewith established formats. Collaborate with other Clearwater Valley and/or St. Mary\'sdepartments i.e., Case Managers, Visiting Nurse Service, RespiratoryTherapy etc., to ensure continuity of care and a seamless transitionback to primary or specialty care. Maintains confidentiality of all hospital and patient information atall times. Follows HIPAA regulations and policies. Regular and predictable attendance is an essential job function. **Minimum Qualificiations:** Master\'s Degree in Social Work, Psychology or related BehaviorHealth; preferred Bachelor\'s Degree, minimum 3-5 years previous relevant experience working in a mental healthfacility or practice is strongly preferred. Licensed as an LCSW, LCPC, LSW, BSW or PhD Psychologist. Full time 30-40 hours per week, weekdays TBDSMH is EOE
