Social Worker / Admissions Assistant

Responsibilities: Assist in the planning, organizing, implementing, evaluating, and directing the provision of social services to the Cedar Lake Health and Rehabilitation Center residents in accordance with current federal, State and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Social Services Director and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. Required Qualifications: Bachelor’s Degree in Social Work or related field (Four Year College or University) Certification by the State of Wisconsin as a Social Worker preferred, but actively working toward certification within 6 months of hire is acceptable.
