This full-time position entitles you to benefits and operates on a Monday to Friday schedule.As a Community Support Specialist, you will provide integrated and holistic community-based support services that aim to foster wellness and recovery.Essential FunctionsClinical Service ProvisionProvide individualized and group education services using a motivational, stage-matched approach, focusing on the client’s strengths, needs, abilities, and preferences.Assist clients in developing a Wellness Plan and support them in achieving their goals.Offer holistic support by incorporating the Eight Dimensions of Wellness.Maintain contact with clients who are hospitalized, engaging in discharge planning and aftercare support.Educate clients on daily living skills, budgeting, and maintaining a residence for independent living.Aid clients in accessing public services and developing personal support systems.Conduct follow-ups regarding appointments and assist with transport as clinically indicated.Integrated Health TeamCommunicate with team members including psychiatrists and nurses regarding client wellness and treatment.Collaborate with clients to identify their service needs and facilitate referrals.Act as a liaison between clients and their support systems, including family and legal authorities.MiscellaneousSeek culturally competent services to meet the unique needs of clients.Perform additional duties as assigned.QualificationsHigh School/GED required.Preferred: Associate’s/Bachelor’s/Master’s degree in a human service-related field.If no degree, a minimum of:Four (4) years of experience or a combination of education and experience if High School/GED is the highest education.Two (2) years of experience or education if Associate’s is the highest.One (1) year of experience if a non-human service Bachelor's degree is obtained.Prior experience working with individuals with psychiatric, substance use, and/or developmental disabilities.Valid driver’s license and agency-required automobile coverage.About UsCompass Health Network is a nonprofit health care organization that provides accessible and compassionate behavioral health services throughout Missouri.
