Whether you are a nurse, caregiver, culinary professional, maintenance worker or looking for a new career, we offer a wide range of career opportunities to fit your skills and interest.The Registered Nurse Assessment Coordinator (RNAC) is accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in skilled nursing.This is a full time ONSITE position in Bethlehem, PA.Qualifications:Active Pennsylvania Registered Nurse licensure requiredRAC-CT certification preferredGerontological Certification preferredBachelor's degree in Nursing preferredActive CPR requiredTwo-years RNAC or related experience preferredKnowledge & Skills:Understanding of Medicare, Managed Care and Medicaid reimbursementsKnowledge of federal and state regulationsExcellent communication skillsComputer skillsKnowledge of MDS 3.0, CAAs, Care Area Triggers, Resource Utilization Groups (RUGs) and Case Mix IndicesUnderstanding of the overall care planning process Responsibilities and Expectations:Oversee and coordinate the entire collaborative assessment and care planning processEnsure appropriate services delivered and available to residentsMaintain organized reports and data using appropriate guidelinesResolve insurance and /or managed care operational issues (authorization, payment level of services, billing/claims payment for referrals and admissions)Monitor rehabilitation therapy utilization for all residents to ensure reimbursement of those services are appropriate and accurateCoordinate Resident Diagnosis CodingServe as educator and resource for team on the documentation of care and services providedAssist with survey and audit duties; preparation of documents based on regulatory agency requestsDevelop positive relationships and be accessible to all residents, families, visitors and team membersPromote and support culture of patient centered care and service
