Key Responsibilities Oversee the execution of ECM, Community Supports, and CHW programs across various locations Manage team performance and ensure adherence to defined workflows and operational consistency Identify and resolve operational issues affecting care delivery and escalate as necessary Required Qualifications Direct experience with CalAIM programs in California 10-12+ years in healthcare operations focused on care delivery or population health 5+ years of experience managing multi-layered teams in a distributed environment Proven track record in program execution and performance management Strong understanding of Medicaid populations and care coordination workflows
