Physical Therapist San Jose, CA and Burlingame, CA We have multiple openings available: Burlingame, Stanford, Forster City, Alameda, Almaden, Willow Glen, Los GatosSalary: $95-140K/yr SUMMARY:Plans and administers physical therapy treatment for patients suffering from injuries or muscle, joint, or bone disease to restore function, relieve pain, and prevent disability by performing the following duties: ESSENTIAL DUTIES AND RESPONSIBILITIESExamines and evaluates patients to determine the nature and extent of the disorder and treatment planReview physician referrals and medical records, if applicableTests and measures patient's strength, motor development, sensory perception, and functional capacity, and records findings to develop or revise plan of carePlans and prepares a written treatment program based on the evaluation of patient dataTreats acute and chronic injuries and related disabilitiesAdministers manual exercises to improve and maintain function and prescribes routine and corrective exercisesUses a variety of exercise techniques including neuromuscular reduction, PNF, PRE, endurance, and balanceInstructs, motivates, and assists patients to perform various physical activities and use of assistive and supportive devices such as crutches and canesInstructs individualized rehabilitation programs including functional exercises, mobility and gait trainingMobilizes and stretches soft tissuesApplies or directs the application of modalities including but not limited to heat, cold, ultrasound, iontophoresis, electrotherapy, and hydrotherapy as needed to promote healingInstructs patients in correct body mechanics for activities of daily living and prevention of injuryTailors home exercise programs based on diagnosis, relevant critical pathways, and functional limitationsProvides education and instruction to patients and their caregivers on the mechanism of injury, safety procedures, home exercise programs, self-care,e and injury preventionRefers patients as needed to appropriate medical specialistsEvaluate the effects of treatment at various stages and adjust the plan of care to achieve maximum benefitAdministers soft tissue and joint mobilizationRecords treatment, response, and progress in electronic medical records according to company policyConfers with physician or other practitioners to obtain additional information, revise plan of care, and integrate physical therapy treatment with other aspects of patient's health careCommunicates with healthcare team members, attorneys, insurance companies, or employers regarding patient progress, as allowed by lawInstructs and directs work activities of physical therapy assistants, aides, and studentsCorrectly completes documentation, charge sheets, progress reports, etc. in a timely mannerComplies with all HIPAA rules and regulations and company policy and proceduresCollaborates and cooperates with business office staff to ensure proper coding and administrative protocols are followed for the purposes of verification, authorization, billing, collections, and record requests QUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
