This role works collaboratively with a dedicated clinical care team to drive positive patient outcomes through accurate assessment, diagnosis, treatment, health counseling, and care planning. The position reports directly to the Practice Manager and requires the ability to practice autonomously while supporting team-based care.Primary ResponsibilitiesProvide primary and family evidence-based care, including assessment, diagnosis, treatment, management of health conditions, health counseling, and disposition planningManage patient appointments and perform routine physical examinationsConduct diagnostic testing and coordinate care with other healthcare professionalsEducate patients on healthy lifestyle choices, chronic condition management, and preventive careEncourage patient engagement through health screenings, wellness services, and behavioral health supportPerform administrative duties including opening and closing the clinic, inventory management, patient follow-up, insurance verification, payment collection, and maintaining a clean and safe clinical environmentCollaborate daily with a clinical care team to ensure quality outcomes across the patient panelRequired QualificationsMaster's Degree from an accredited Family Nurse Practitioner programCurrent National Board CertificationActive state license to practice as an Advanced Practice NurseCurrent BLS certification from the American Heart Association (AHA) or American Red Cross (ARC)Active BLS must be provided upon offer acceptanceVerifiable High School Diploma or GEDMinimum of 1 year of recent primary care experience as a Nurse Practitioner in a family medicine or primary care settingOR successful completion of primary care training programWorking knowledge of quality measurement in chronic disease managementAbility to work autonomously without direct supervisionStrong clinical judgment, initiative, and patient advocacy skillsEffective verbal, written, and electronic communication skillsStrong organizational skills and ability to multitaskProficiency with healthcare information systems and technologyAbility to collaborate frequently with interdisciplinary healthcare professionalsWillingness to obtain multi-state licensure as neededAbility to work within a remote clinical care team culture when applicableAdditional InformationWeekend and holiday rotations may be requiredCandidate questionnaire must be included on the resumeINDKS
