(eligible after 1yr or 1000 hours worked)</li></ul><p><br/></p><p><strong>Our Culture</strong></p><ul><li>Leadership is available to confer, consult and advise at any time</li><li>You are encouraged to speak professionally and freely</li><li>Work-life balance is a top priority</li><li>Laughter, passion for personable-care, and a supportive environment</li></ul><p><br/></p><p><strong>RN/LPN Responsibilities</strong></p><ul><li>Complete ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family</li><li>Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician</li><li>Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment</li><li>Ensures that equipment, supplies, drugs and biologicals, and other necessary items and services are available</li><li>Make necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change</li><li>Completes accurate and timely documentation of patient/family services provided</li><li>Regularly attends IDG meetings</li></ul><p><strong>RN/LPN Requirements</strong></p><ul><li>RN/LPN with active, unrestricted license in the State of Michigan</li><li>At least one (1) year of relevant experience as an RN or LPN (required)</li><li>One (1) year of management experience (preferred)</li><li>One (1) year of home health experience (preferred)</li><li>Current CPR Certification</li><li>Community health and a wide range of Medical-Surgical experience (preferred)</li><li>Reliable transportation with a valid state driver's license and current car insurance</li></ul><p><br/></p><p>#Homecare</p><p><br/></p><p><strong>Interim HealthCare</strong> is an Equal Opportunity Employer.</p>Interim Healthcare - West Michigan.
