Targeted Case Manager/ Medical Staff Associate Dual Role - Somerset, KY
2nd Chance Clinic is an outpatient addiction treatment center in Somerset, KY. Our Headquarters is located in Lexington Kentucky. We are currently seeking a full time employee in a dual role as a Targeted Case Manager and Medical Staff Associate for our Somerset office.The following are a list of responsibilities and dutiesComplete new patient assessments. Complete new patient biopsychosocial assessments and preliminary screeners, and review client's strengths and needs. Determine level of care need from assessment and discuss with patient to ensure patient's understanding. Evaluate with patient that care needs meet their basic health, safety and overall welfare needs. Ensure that patients understand their rights regarding confidentialityPlan services based on assessments. Facilitate access, quality, and delivery of necessary services and provide follow-up to patient. Provide psychoeducation and resource information while acting as a positive role model, providing patients with guidance and support. Promote and foster effective relationships, both inside and with outside agencies for continuity of care. Ensure that documentation is complete, accurate, timely and meets regulatory and agency standardsMaintain client confidentiality in accordance with federal, state, and agency standards. Practice ethical standardsRespond to all patient grievances in accordance with written policiesParticipate in quality assurance and utilization reviewAttend quarterly all staff meetings and monthly departmental meetingsOffice organization and administrationPerform observed urine collectionPerform routine venipuncture (if trained)Adhering to regulations and legislationInterview patients and document basic medical historyOrganize and schedule appointmentsUpdate and file medical records and insurance prior authorizationsArrange for laboratory servicesCollate patient charts and complete appropriate documentationPerform intake examinationsLiaise with other organizations regarding insurance, patient care plan, or documentsHandle accounts receivable and produce daily balance sheetPrepare and clean clinic and treatment roomsInventory supplies for purchasingOrganize, track and prepare encounter forms for billingReport perceived issues to supervisors immediatelyAssist physicians with administrative dutiesOther duties as assignedRequirements Bachelor's degree in human services field plus 2 years' experience in behavioral health case management (or an equivalent of education and experience)Provide information for a complete background checkCurrent certification in BLS, non-violent crisis intervention, and suicide intervention a plusExcellent oral and written communication skills, basic computer skills in word processing, email and Internet use, strong interpersonal skills, and professionalismMaintain required continuing education units per state regulations for targeted case managersComplete Targeted Case Management Core training and SUD training provided by an approved provider of the Department of Behavioral Health, Developmental and Intellectual Disabilities within 6 months of employment.
