This role is responsible for building strong partnerships within contracted healthcare facilities, identifying potential hospice referrals, and facilitating timely admissions into hospice care.The Clinical Liaison provides education and training to healthcare facility staff on hospice services, pain and symptom management, and end-of-life care. This position also supports patients and families by explaining hospice benefits and coordinating admissions in collaboration with the admissions team.Key ResponsibilitiesBuild and maintain relationships with contracted nursing facilities and referral sources Identify and coordinate hospice referrals and facilitate smooth admissionsProvide education and in-services on hospice care, pain management, and end-of-life issuesServe as a resource to facility staff, physicians, and community partnersSupport care coordination for patients transitioning between care settingsEnsure accurate and collaborative documentation in facility medical recordsParticipate in community outreach and professional engagement activities - including creating and presenting community presentations QualificationsGraduate of an approved school of nursing (BSN preferred)Current Registered Nurse (RN) license in the State of TexasKnowledge of hospice and nursing facility regulationsExcellent communication, presentation, and interpersonal skillsSelf-motivated with the ability to work independentlyValid Texas driver’s license, reliable transportation, and auto insuranceWorking ConditionsThis position involves frequent travel to contracted facilities, flexible hours, and participation in an on-call rotation (1-3 times per quarter).Please note this job description has been designated to include the general nature and level of work performed by employees in this position.
