Work Location: In the homes and community of individuals served - serving Chenango CountyPosition Summary: The purpose of ACT (Assertive Community Treatment) is to meet the needs of individuals who have not been well served by traditional service delivery approaches. Grounded in a recovery-oriented practice, ACT offers evidence-based treatment, rehabilitation, case management, and support services through a mobile, multidisciplinary behavioral health treatment team.The Team Leader will play a pivotal role in providing direct clinical services, supervising specialty areas based on evidence-based practices, trauma-informed care, cultural humility, and person-centered language, as well as offering administrative oversight to the team.ResponsibilitiesProvide direct clinical services to individuals with Serious Mental Illness (SMI).Supervise each specialty area within the team, ensuring adherence to evidence-based practices, trauma-informed care, cultural humility, and person-centered language.Offer administrative oversight to the ACT team, including scheduling, resource allocation, and program development.Conduct regular individual supervision sessions with team members.Lead team meetings at least four times per week to review client statuses, discuss treatment plans, and coordinate care.Requirements
Education (one of the following)Master's degree requiredLicensed Professional: LMSW, LCSW, LMHC, LMHP, LCAT, LMFT or related license requiredPhysician or Physician AssistantPsychiatric Nurse Practitioner (PMHNP)Registered Nurse (RN)PsychologistExperienceExperience working on a multidisciplinary mobile team is preferred.Strong understanding of evidence-based practices, trauma-informed care, cultural humility, and person-centered language.Excellent leadership, communication, and organizational skills.Driver's License and ability to maintain insurability throughout employment required.Benefits
