We are growing thoughtfully and are excited to expand our clinical footprint, with priority opportunities in Washington and future openings in MI, GA, MI, FL, OH, VA, and WA.About the RoleThe Health Guide (Nurse Practitioner) plays a critical role in leading the medical aspects of virtual, whole-person care for individuals with serious mental illness supported by firsthand. This role is Washington-focused, supporting individuals across WA while collaborating closely with community partners and internal care teams. You will conduct comprehensive virtual assessments, manage acute and chronic conditions, and collaborate closely with:Community primary care and behavioral health providersAn internal team of triage RNs, Psychiatric Nurse Practitioners, and support staff This is a highly collaborative, mission-driven role designed for clinicians who thrive in virtual care environments and are passionate about serving vulnerable populations.ResponsibilitiesVirtual Whole-Person Clinical Care Conduct comprehensive virtual assessments addressing both behavioral and physical healthObtain thorough histories and perform virtual physical examinationsDiagnose and manage acute and chronic conditions, including prescribing medications (short- and long-term)Identify previously undiagnosed conditions and order appropriate diagnostic studiesCollaborate with medical records specialists, documentation teams, certified coders, and medical assistantsDevelop individualized care plans, referrals, and recommendations aligned with patient goalsSupport self-management skills using motivational interviewing and holistic care planningOrganize clinical documentation to support continuity of care and disability applications (when appropriate)Unscheduled & Transitional Virtual Care Provide high-quality, compassionate care with a commitment to maintaining NPS ≥ 90%Support efforts to reduce ER utilization within 7 days of patient interactionDeliver urgent virtual care for conditions including: COPD and CHF exacerbationsUncontrolled diabetesCold/flu symptoms, UTIs, painAnxiety, depression, and suicidal ideationConduct transition-of-care visits within 7 days of ER or hospital dischargeTeam Leadership & Clinical Support Lead individual progress reviews in partnership with firsthand GuidesSupport in-person Health Guides and centralized teamsAssist with implementation of new interventions, workflows, and care processes Qualifications3+ years of clinical experience as a Nurse Practitioner (Primary Care, Emergency Medicine, or complex care settings preferred)Active Washington NP license requiredWillingness to obtain additional licenses in GA, FL, MI, OH, VA, and WA as the organization continues to growRequired SkillsDeep passion for serving individuals living with serious mental illnessStrong sense of empathy, compassion, and emotional intelligenceTeam-oriented mindset with a positive, collaborative approachComfort with technology and virtual care deliveryInterest in data-driven, outcomes-focused careAdaptability in fast-growing, evolving care environmentsExcellent listening, communication, and interpersonal skillsAbility to maintain professional boundaries while fostering meaningful engagementWillingness to become certified in Psychiatric Nursing (RN level) Pay range and compensation packageCompetitive compensation aligned with experience and licensureComprehensive healthcare benefits for you and your family401(k) with employer matchFlexible and competitive paid time offChoice of Mac or PC laptop or tablet Equal Opportunity StatementWe firmly believe that great candidates may not meet 100% of the criteria listed above.
