In this role, you will help patients improve mobility, function, and overall quality of life by implementing individualized care plans.What You'll DoDeliver physical therapy treatments as directed by the supervising PTPerform therapeutic exercises, mobility training, and transfer assistanceUtilize modalities such as heat, cold, electrical stimulation, and tractionAdjust treatments within scope based on patient responseObserve and document patient response to treatmentReport changes in condition, pain levels, or safety concerns to the PTCollaborate with the care team to support positive outcomesTeach home exercise programs and safety techniquesSupport patients and caregivers in following the plan of careComplete accurate, timely documentation in EMREnsure compliance with Medicare, Medicaid, and agency guidelinesParticipate in quality improvement initiativesFollow infection control and safety protocols in the homeMaintain HIPAA compliance and patient confidentialityAttend trainings and meet continuing education requirementsAssociate degree from an accredited PTA programActive, unrestricted Colorado PTA licenseCurrent CPR certificationValid driver’s license, reliable transportation, and auto insuranceRequirementsAssociate degree from an accredited PTA programActive, unrestricted Colorado PTA licenseCurrent CPR certificationValid driver’s license, reliable transportation, and auto insurancePreferred1+ year of PTA experienceHome health experience
