They are currently hiring due to growth.POSITION DESCRIPTION: This position will provide a full scope of primary care services including, but not limited to, the diagnosis, treatment, coordination of care, preventive care, and health maintenance for patients. This role is open to both internal medicine and family medicine providers and an appropriate patient panel will be created.Specific duties of the role include, but are not limited to:- review patient histories and perform physical examinations- evaluate, diagnose and provide appropriate treatment and patient care- refer patients to specialists as needed- provide quality clinical diagnostic skills in the delivery of patient care- review all test results and recommend suitable management for patients- prescribe medications- formulate ongoing treatment and disease management plans- provide relationship-centered care, taking into account psychosocial and physical needs- collect and record patient information, such as histories, reports, and examination results- communicate with the clinical team to ensure total delivery of quality care- participate in medical staff educational programs and meetingsPhysicians see an average of 22 to 23 patients per day with the support of dedicated clinical assistants and administrative team members.The practice uses the Epic EHR.REQUIREMENTS:- New York medical license- DEA- Board Certification or Eligibility, Family Medicine or Internal Medicine- 2+ years outpatient primary care experience requiredHOW TO APPLY:To apply for this position, please send your resume to Jenn Kunkel at or call .Please visit for a full list of openings at CHS Recruiting.
