Licensed Practical Nurse (LPN) – Wound VAC Care (Wednesdays & Fridays) 19401

We are hiring a skilled Licensed Practical Nurse (LPN) to provide Wound VAC therapy and wound care support for a home health patient requiring services twice weekly- Wednesdays and Fridays. This is a consistent, ongoing assignment ideal for an LPN with strong wound care skills, excellent assessment abilities, and confidence working independently.Key Responsibilities Perform Wound VAC dressing changes and equipment management in accordance with physician orders and agency protocols. Conduct wound assessments, identify signs of infection or complications, and report changes to the RN Case Manager or physician. Provide education to the patient and/or caregivers regarding wound care, equipment use, and infection prevention. Accurately document all care in Axxess (or EMR provided) on the same day of service. Communicate professionally with the care team to ensure continuity of care and timely followup. Adhere to CHAP standards, state regulations, and agency policies. Qualifications Active Pennsylvania LPN license in good standing. Wound VAC experience required Previous home health or facilitybased wound care experience strongly preferred. Strong attention to detail, good communication skills, and reliability. Must be available every Wednesday and Friday.
