Intensive Outpatient Program Coordinator

Overview: This position requires the full understanding and active participation in fulfilling the mission of Tara Treatment Center, Inc. The Program Coordinator is responsible for coordinating all aspects of the assigned program. This includes direct client care. Essential Duties and Responsibilities: Conduct confidential, quality assessments to determine the individuals’ need for alcohol and drug services and evaluate their suitability for participation in a specific level of care. Work closely with clients to engage them in the decision-making process and help them understand the benefits of treatment and accessing the recommended level of care. Collaborate with the client and the multi-disciplinary treatment team to develop an individualized treatment plan with measureable goals. The clients’ needs and short/long term goals should be reflected in the treatment plan. Review and evaluate clients’ progress in relation to measureable goals identified in the treatment plan. Modify treatment plan to accommodate changes in clients’ status. Develop and maintain productive relationships with referral sources. Provide regular updates regarding treatment progress, recommendations, discharge plans, and other relevant information specific to the client they referred. Use a strengths-based and trauma informed approach to work collaboratively with outside resources to ensure coordination of services when necessary. These may include, but are not limited to the criminal justice system and agencies specializing in the care of individuals’ with a history of trauma, abuse, neglect or exploitation. Consult with supervisor or other members of the multi-disciplinary treatment team as needed. Participate in all assigned meetings, staff development, and training as required. Complete paperwork requirements and maintain quality, up to date clinical records. Conduct group therapy, psycho-education groups, and individual therapy and provide back-up assistance with group therapy and psycho-education groups to other counselors and support staff as necessary. · Maintain an active role in the development and implementation of performance improvement initiatives. Remain in compliance with internal and external policies, procedures, regulations, and standards. Participate in continuing professional development. Minimum Skills and Qualifications: A Master’s degree in social work, counseling or related field preferred. One year of clinical experience in alcohol and drug treatment preferred. Current CPR certification. Scheduling flexibility, holiday and weekend rotation is required. · A valid Indiana driver’s license or a valid picture identification issued by a state or federal agency. Competencies: Ability to establish rapport. Apply accepted criteria for diagnosis of substance use disorders. Determine the readiness of the individual for treatment and change. Ability to systematically gather data. Screen for danger to self or others by completing the pre-screening assessment, suicide risk screening tool and when necessary the suicide risk assessment. Ability to screen for co-occurring mental health issues. Ability to analyze and interpret data to determine treatment recommendations and priorities. This includes Trac 9 findings. Collaborate with the client to formulate mutually agreed upon, measurable treatment goals and objectives. Demonstrate adherence to accepted ethical and behavioral standards of conduct. Knowledge and application of DSM-V diagnostic and ASAM criteria. Knowledge of 12-step programs and recovery support systems. Strong interpersonal and engagement skills. Effective written and verbal communication skills. Ability to develop an effective safety plan and/or behavioral contract. Ability to oversee and accurately document self-administered medications.
