Inspiration Hospice is seeking a dedicated and compassionate Licensed Medical Social Worker to join our team. Inspiration is a fast-growing hospice agency offering a great work environment, excellent benefits. In this role, you will provide emotional support, counseling, and resources to patients and their families during their hospice journey. Your expertise will help enhance the quality of life for those we serve, making a meaningful impact in their final stages of life.
Key Responsibilities:
Attends all IDG and mandatory meeting
Conduct comprehensive assessments of patients and families to identify needs and develop appropriate care plans
Provide emotional support and counseling to patients and their families, addressing grief, loss, and adjustment issues
Collaborate with interdisciplinary team members to ensure holistic care and support for patients.
Facilitate communication between patients, families, and healthcare providers to ensure continuity of care
Connect patients and families with community resources and support services as needed.
Maintain accurate and timely documentation of patient interactions and care such as assessment of emotional factors, counseling for long-range planning and decision-making, community resource planning, short term therapy, etc.
Required Qualifications:
Master’s degree in social work (MSW) from an accredited program
Current state licensure as a Licensed Social Worker or equivalent
Experience in hospice, palliative care, or a related field preferred
Strong interpersonal and communication skills, with the ability to empathize and connect with patients and families
Ability to work collaboratively within a multidisciplinary team
Knowledge of community resources and support services for patients and families
Must past background check & drug screen
Reliable transportation is required
