Director of Case Management

Community hospital part of a system looking to bring on Director Case Management! Bonus Incentives and Relocation! CCM or ACM within the first year of hire and there will be a 2k increase in pay, hospital will pay for certification Open to RN or Social Worker (for nursing, must have BSN with RN licensure) for other eligible health care professionals, must have a minimum of a Bachelor’s degree At least 1 year of hospital based case management experience including utilization review and discharge planning experience Direct reports of 3 full-time case managers, 1 case manager assistant, 2-3 per diem case managersBreakdown of direct reports: 2 RNs, 1 Physical Therapist, 1 per diem is RN, 1 per diem is Social WorkerCase load goal is 12-13 patients
