Registered Nurse (RN) Hospice Home Health Case ManagerWe are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join a dedicated hospice team serving patients in the Burlington and Walworth communities.The RN Case Manager will coordinate and manage care for patients with end-stage illnesses, ensuring comfort-focused care in various home-based settings. This role requires strong clinical judgment, excellent communication skills, and the ability to provide holistic care to patients and their families.Patient Population:End-stage hospice patientsCare provided in:Patient homesAssisted Living Facilities (ALF)Skilled Nursing Facilities (SNF)Group homesIndependent living communitiesTwo hospitals providing General Inpatient (GIP) hospice careUnit Overview:Average Daily Census: ~105 patientsRN Case Managers: 7 (each managing 1416 patients)CNA Support: 6 CNAsKey Responsibilities:Manage a caseload of hospice patients in the communityProvide comprehensive nursing assessments and symptom managementDevelop and implement individualized care plansCoordinate care with physicians, hospice staff, and interdisciplinary teamsEducate patients and families regarding disease progression and end-of-life careDocument patient care accurately in the EMR systemProvide compassionate support to patients and families during end-of-life careRequirements:Minimum Experience: 2 years RN experienceTravelers: Experienced travelers preferredLicensure: Active Registered Nurse (RN) licenseCertifications:BLS (Basic Life Support)Other Requirements:Valid Driver's LicenseAuto InsuranceReliable transportation for community visitsSkills & Competencies:Hospice or end-of-life care experience preferredStrong patient assessment and care coordination skillsAbility to work independently in a community-based settingExcellent communication and compassionate patient careTechnology & Equipment:EMR: Epic & RoverIV Pump: YesMedication Dispensing: YesMonitoring Equipment: Chair and bed alarmsSupplies: Home health bag and trunk suppliesScheduling & Work Environment:Shift: Monday Friday, 8:00 AM 4:30 PMWeekend Requirement: NoneOn-Call: NoneHoliday Requirement: NoneFloating: Community-based patient home visitsOrientation: 1 weekAdditional Information:Scrub Color: Blue or BlackParking: Free parking available at Burlington and Walworth officesUnit Culture: Strong teamwork and supportive environment
