Responsible for meeting established standards of skilled nursing practice, the LPN collaborates with the care team and participates in interdisciplinary care to advocate for patients. The LPN effectively provides person-centered care and services while administering medications and treatments under the supervision of a Registered Professional Nurse in accordance with facility policy. ResponsibilitiesMonitor and record patient vital signs such as respiration, temperature, blood pressure, and heart rate.Report changes in patient conditions or other pertinent observations to other nursing personnel and the responsible RN.Coordinate care with the interdisciplinary team to ensure effective patient care.Maintain clinical competence specific to the care area and complete required education within the stated timeframe.Document appropriately and thoroughly in the Electronic Medical Record (EMR) / Point Click Care.Adhere to ethical and professional standards and advocate for patients.Float to other units as staffing permits and may be needed to drop to CNA position as necessary. Schedule This is a 24-hour position requiring three evening shifts per week from 3:00 PM to 11:30 PM. Every other weekend and holiday is mandatory. Required Experience / Certifications / LicensureCurrent License as a Practical Nurse in the state of practice.Experience with Point Click Care is preferred.Completion of necessary health screenings and immunizations as outlined in the personal health history.
