Extended hours and occasional weekends may be required as needed. About the Role The Clinical Pharmacist plays a key role in delivering telephonic consultations to members, focusing on comprehensive medication reviews, and improving disease state management. Key Responsibilities Conduct telephonic consultations and schedule appointments for members enrolled in clinical programs. Utilize prescription claims data to provide clinical recommendations under MTM and Retrospective Drug Utilization Review protocols. Identify gaps in care, high-risk medications, compliance/adherence issues, drug interactions, and opportunities for therapeutic or generic substitution. Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions. Educate patients on medication regimens and disease states. Document patient care notes and recommendations accurately and professionally. Stay current with emerging clinical trends and participate in departmental meetings. Meet productivity and performance expectations. Support special projects and other duties as assigned.
