*PRIMARY JOB DUTIES* 1. Provide total care and/or assist with personal hygiene, elimination, feeding, and mobility according to the patient’s needs and the nursing instruction as identified in the plan of care 2. Recognize and report any changes in the mental or physical status of the patient promptly to the RN Case Manager or Hospice Clinical Manager. Be aware of potential anxiety in families. 3. Help the patient achieve a good quality of life through: a. Encouragement of independence, eating, social contact, etc. b. Providing companionship c. Providing reassurance 4. Carry out the duties assigned in order to increase the patient’s level of comfort, and to maintain personal hygiene and a safe environment for the patient. 5. Demonstrates positive interpersonal relations in dealing with all members of the agency. 6. Effectively demonstrates the mission, vision and values of the Agency on a daily basis. *1.0 40% QUALITY OF WORK* 1.1 20% Participating in patient’s plan of care to meet patient’s needs and promote comfort. * Observing the patients’ and family/caregivers’ physical, psychosocial, bereavement, environmental, safety, and developmental needs, and reporting to the interdisciplinary team for follow up. * Implementing the Hospice Plan of Care as prescribed by the physician, under the supervision of the Registered Nurse, and in conjunction with the other members of the interdisciplinary group. * Providing care to patients and families through the utilization of interventions as indicated on the patient’s plan of care. * Assisting the RN Case Manager and other interdisciplinary team members in the revision of the plan of care by reporting observations and changes. * Provide support, instruction, and education of the patient, family and other caregivers who participate in the care of the patient in relation to promoting patient’s comfort and providing for patient’s personal needs. 1.2 10% Effectively and efficiently assists in the care of hospice patients. * Maintains open communication and coordination of care with other members of the interdisciplinary team. * Maintains an accurate and updated medical record, documenting care and interventions as indicated by patient’s plan of care. * Attends and participates in regularly scheduled interdisciplinary group meetings, as schedule permits. * Reports abuse and neglect in accordance with state laws and regulations as well as organization policy and procedure. 1.3 10% Contributes to program effectiveness as evidenced by: * Provides compassionate and patient/family-centered care to hospice patients and their families. * Maintains certifications as required by the state and federal regulations, as well as participated in continuing education and required annual training. * Actively participates in quality assessment of performance improvement teams and activities. *2.0 40% EFFICIENCY AND EFFECTIVENESS:* 2.1 30% Organizes and performs work effectively and efficiently as evidenced by: * Demonstrates understanding and compliance with agency policies, including but not limited to, documentation, continuing education, attendance/work schedules and required HR documentation, State and Federal policies and procedures, laws and regulations. * Taking only supplies that are needed and is conscientious of minimizing on-hand inventory. * Works with diverse social and economic situations without imposing own values; maintains appropriate professional boundaries * Practicing personal cost containment by responsible use of equipment, supplies, and resources. 2.2 10% Maintains and adjusts schedule to enhance team performance as evidenced by: * Reporting to work on time and returning promptly from errands, breaks, and meals. * Managing personal work schedule and time off to promote smooth agency operations. * Assisting other team members to ensure completion of all work assignments. * Demonstrating flexibility with changing workload/assignments. * Does not exceed accrued PTO or qualified leave of absence(s).* *3.0 20% TEAM WORK, MISSION, VISION, VALUES:* 3.1 20% Demonstrates positive interpersonal relations in dealing with all members of the team (i.e. co-workers, supervisors, physicians, etc.) as evidenced by: * Responding to all customers in a courteous, sensitive and respectful manner. * Demonstrating respect for team members. * Managing stress and personal feelings without a negative impact on the team * Working collaboratively and cooperating with other departments. * Completing the review period without any formal disciplinary action.* * Presenting a clean and neat appearance in personal attire and one’s work area. * Cannot exceed *JOB SPECIFICATIONS* 1. Education: Completion of certificate program in nursing care from an accredited training program with current listing as CNA-1 or CNA-2 2. Licensure / Certifications: CNA-1 or CNA-2 and current American Heart Association CPR certification required. Must have current NC driver’s license with an automobile that is insured in accordance with state or organization requirements. 3. Experience: 6 months experience required; hospice experience preferred. 4. Essential Technical/Motor Skills: Understanding various medical conditions and terminology, proficiency in basic computer applications such as Microsoft Office applications. 5. Interpersonal Skills: Exhibit an attitude which promotes harmony and goodwill in the workplace. Excellent communication skills; both oral and written. 6. Essential Physical Requirements: Assisting patients with activities of daily living that require frequent pushing, moving, lifting of patients. Frequent positioning of patients, giving baths, ambulating patients requiring much physical effort. Must be able to use hands to handle, feel and grasp. Must be able to use proper body mechanics to lift at least 50 pounds. Requires the ability to stand, walk and sit. Must be able to climb stairs, stoop, kneel and crouch. May require aide to independently push or pull a patient weighing more than 100 lbs. 7. Essential Mental Abilities: The ability to follow detailed and diverse instructions for assisting patients in the daily living routine. Requires long and short-term memory. Must be able to create and follow a daily schedule of visits. 8. Essential Sensory Requirements: Keep patient’s area clean and orderly. Utilize vision to assess and care for patients safely and to document information accurately. Utilize hearing to hear alarms and beepers, effectively communicate with patients, families, and staff. Utilize sense of smell to detect patient changes. 9. Exposure to Hazards: Frequent exposure to blood and body fluids, infectious diseases. May be exposed to dangerous animals and traffic hazards while home visiting. May encounter situations which present a potential threat to personal safety. May encounter temperature changes and weather extremes. 10. Other - Hours of Work: 40 hour week, 8:00 am - 5:00 pm, Monday - Friday. Overtime may be required during peak workloads or increase in volume. 11. *Job Limitations – *The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician’s orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques.5c143e31-5e48-4549-b638-05792d185386
