Employment TypeFull-time | Contract-to-hire (6–12 month initial engagement with potential extension or conversion)Role OverviewWe are seeking a licensed Nurse Practitioner (NP) to join a technology-enabled, value-based care program focused on delivering high-quality, patient-centered care to Medicare Advantage populations.In this field-based role, you will conduct in-home and community-based health assessments, identify care gaps, and support proactive chronic disease management for older adult patients.This is a highly autonomous clinical position designed for providers who are comfortable working independently in the field while coordinating closely with virtual care teams.Key ResponsibilitiesConduct comprehensive in-home health assessments and annual wellness visitsPerform medication reconciliation and focused physical assessmentsIdentify and document diagnoses to support risk adjustment (HCC coding)Develop individualized care plans based on patient needsIdentify social determinants of health and connect patients to community resourcesCoordinate care with primary care providers, specialists, and care teamsSupport transitions of care and follow-up for high-risk patientsDocument all encounters accurately and within required timeframesRecognize and escalate urgent or acute medical concerns when neededMeet productivity and quality metrics (visit volume, documentation accuracy, turnaround time)Patient PopulationPrimarily Medicare Advantage membersGeriatric and medically complex patientsChronic conditions such as diabetes, CHF, COPD, hypertension, dementia, etc.Patients requiring preventive care and ongoing care coordinationRequired QualificationsActive, unrestricted Nurse Practitioner or Physician Assistant license (California)National board certification (AANP, ANCC, or NCCPA)Active DEA registration or eligibility to obtainCurrent BLS / CPR certificationValid driver’s license, auto insurance, and reliable personal vehicleAbility to travel throughout assigned territory for daily home visitsComfortable working independently in patient homesProficiency with EMR systems and digital documentation
