How you’ll make an impact in this role Lead the clinical journey by implementing and adjusting patient care plans in real-time, ensuring every intervention is purposeful and aligned with the patient’s recovery goals. Create a seamless care experience by serving as the primary coordinator across all medical disciplines, ensuring that physical therapy, pharmacy, and specialty consults are perfectly synchronized for the patient. Support patient safety and advocacy by diligently monitoring clinical conditions and communicating vital changes to the physician team, ensuring that every patient’s "voice" is heard through accurate data and observation. Empower patients for life after discharge by assessing and coordinating their transition home from day one, collaborating with the healthcare team to ensure a safe, successful, and well-prepared return to the community. Execute physician and nursing orders with a focus on clinical excellence, maintaining the highest standards of safety and protocol to ensure the best possible patient outcomes. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
