Together, we can make a profound difference in the lives of those we serve!ResponsibilitiesIn the Social Worker and Therapist Hybrid role, you will utilize both your case management and therapeutic skills to support our Adolescent Partial Hospitalization Program (PHP) and Intensive Outpatient Programs (IOP).Responsible to assess, plan, implement, and evaluate the psychosocial services for patients of PrairieCare. The employee provides a comprehensive range of services to patients and their families, including case management and therapeutic services, and seeks to enhance the functioning of patients. This position may work with children, adolescents, and adults and serving patients with co-occurring disorders.Responsibilities and Accountabilities:As a social worker, have knowledge and understanding of Social Work practices and principlesResponsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and/or therapeutic groupsResponsible for communicating with patients and families to keep them involved in treatment plan decisionsCoordinates care with outpatient providers upon admission and dischargeContributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.As a therapist, facilitates group, individual, and family therapy sessionsWill be available to provide coverage for other social workers/therapists when absent or as the need arisesCommunicate clearly and effectively with clinical team members and DirectorsMay be assigned supervision responsibilities for those pursuing clinical licensureIs oriented and educated in gender, cultural differences, and age appropriate care for the assigned departmentMust be flexible to work at different sites within the PrairieCare metro locations. Qualifications Requires a Master's degree in Social WorkLicensure: Candidates must be licensed LGSW or LICSW in the state of Minnesota.Two years previous mental health experience strongly preferred. Experience in intensive psychiatric healthcare settings preferredExperience using electronic health records preferredCultural responsiveness and bilingual language skills strongly preferredAbility to maintain required malpractice insuranceProficient in Microsoft Office products including Outlook and Word.Able to effectively and efficiently communicate in written and verbal formAbility to smoothly and rapidly shift between dissimilar tasksAbility to communicate well both verbally and in writingAbility to react quickly and calmly as part of a team to handle volatile and potentially emergency situationsAbility to make effective decisions while in stressful situations and circumstancesAfter training, be able to manage disruptive and explosive patients. Compensation for this role ranges from $30- $38 per hour depending on licensure and experience. We are committed to providing competitive and equitable compensation. An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions.
