Family Nurse Practitioner

1. The main function of the Nurse Practitioner will be to provide medical support and coordination to patients and family members. Provides medical expertise on pain, symptom management, and the evaluation of terminal prognosis to patient, family members and staff. 2. Ensures that the care being provided is based on established hospice guidelines and standards. 3. Regularly contacts and collaborates with Acacia Hospice Care providers regarding new patients. 4. Interacts with attending physicians as necessary regarding the care and treatment of patients in the hospice program and works collaboratively regarding potential referrals to hospice. 5. Acts as the attending physician for identified hospices patients. 6. Provides face-to-face encounters for the appropriate hospice patients. 7. Acts as a liaison with other physicians in the community serviced by Hospice and facilitates communication. 8. Participates in educational programs for staff, when requested. 9. Participates in designated interdisciplinary group activities as a member. 10. Provides advice, guidance, and assistance to Hospice staff until a satisfactory resolution is reached when a medical order: Is of a questionable nature. Contains a discrepancy. Lacks clarity. Continues to be a concern for the staff after consultation with the primary physician. Provides medical consultation and direction when the attending physician or their designee cannot be reached and there is a change in the patient’s condition requiring medical attention.
