Charming Lakes Rehabilitation - Position: Travel Physical TherapistLocation: Travel between multiple Skilled Nursing Facilities throughout Lakeland, FLSchedule: Full-TimeSIGN-ON BONUS AVAILABLEPay Rate: Up to $120,000 annuallyPosition SummaryWe are seeking a motivated and experienced Travel Physical Therapist to provide high-quality rehabilitative services across multiple Skilled Nursing Facilities throughout Lakeland, FL. This role is ideal for a flexible clinician who enjoys variety, collaboration, and making a meaningful impact across multiple care settings.The Travel Physical Therapist will be responsible for planning, developing, implementing, and directing physical therapy services while ensuring compliance with all federal and state regulations, facility policies, and professional standards. Our ideal candidate is passionate about resident-centered care and committed to improving functional outcomes and quality of life.Key ResponsibilitiesProvide and oversee physical therapy services across assigned Skilled Nursing Facilities in accordance with physician orders, APTA standards, and Florida practice acts.Plan, organize, and monitor physical therapy programs to ensure regulatory compliance and clinical excellence.Track, analyze, and report key performance indicators (KPIs) related to therapy outcomes and take action to improve performance.Supervise and evaluate Physical Therapy Assistants, ensuring accountability and adherence to professional standards.Conduct resident screenings, evaluations, treatment planning, discharge planning, and accurate documentation.Provide detailed clinical data for assessments, including functional status, therapy minutes, goals, range of motion, balance, and progress.Communicate effectively with residents, families, and interdisciplinary team members to promote positive outcomes.Ensure proper use, maintenance, sanitation, and safety of therapy equipment and treatment areas.Deliver compassionate, resident-centered care that enhances mobility, independence, and overall quality of life.QualificationsActive, unrestricted Florida Physical Therapist license.Graduate of an accredited Physical Therapy program meeting state licensure requirements.Strong knowledge of Skilled Nursing Facility regulations and long-term care standards.Ability and willingness to travel between Lakeland-area facilities.Excellent leadership, organizational, and communication skills.Benefits401(k) Retirement PlanGenerous Paid Time Off (PTO)Comprehensive Medical & Dental InsuranceTeam-Oriented, Supportive Work EnvironmentCompany PerksEmployee Recognition ProgramsTake the next step in your therapy career and join a supportive organization that values excellence, flexibility, and growth.
