Clinical Dietitian - Outpatient (Full-Time or Part-Time)

Job Summary:The Clinical Dietitian - Outpatient is responsible for providing comprehensive outpatient nutrition assessment, counseling, education, and follow-up services for patients based on referrals from medical providers within an ambulatory care setting. Responsibilities include, but are not limited to, chart review, collection and analysis of biochemical and anthropometric measurements, dietary and medical history assessment, and development of individualized nutrition care plans. The position supports patient-centered care and collaborates with interdisciplinary teams to promote optimal health outcomes in a community health environment.Benefits offered: In addition to your base pay, you are also eligible to receive:Generous paid time offBirthday holiday and 7 paid holidaysMedical, Dental & Vision CoverageCompany-paid life insurance.Retirement Plan with matching opportunity$2000 sign-on bonus- paid in 2 separate paymentsEssential Duties and Responsibilities:* Develops, implements, and follows up on individualized outpatient nutrition care plans based on comprehensive assessment and evaluation of nutritional needs, coordinating care plans with other healthcare services as appropriate.* Provides nutrition counseling and medical nutrition therapy to adult and prenatal patients in an ambulatory setting.* Ensures that all referred prenatal clients are instructed on the importance of appropriate weight gain and special dietary needs during pregnancy. This includes plotting patients' weight gain, reviewing vitamin and mineral supplementation, and providing education on substance avoidance during pregnancy.* Documents pertinent data regarding nutrition care plans, identified problems, diet orders, and evaluations in the patient's electronic medical record while maintaining confidentiality and compliance standards.* Provides current, evidence-based nutrition education concepts and procedures to patients, including instruction on appropriate diets in the absence of a provider-ordered therapeutic diet.* Provides disease-specific nutrition education and counseling for patients with conditions including, but not limited to, cardiovascular disease, hypertension, obesity, HIV/AIDS, gastrointestinal disorders, failure to thrive, and liver disease.* Participates as an active member of the interdisciplinary care team and serves as a nutrition consultant. Coordinates with providers, pharmacists, nursing staff, social workers, and other Hamilton Health Center staff as appropriate.* Develops and obtains appropriate educational and instructional materials consistent with outpatient scope of care.* Performs other related duties within the scope of practice as assigned.Required Education/Certifications:Bachelor's in Science degree in Dietetics, nutrition, and related fieldsEducational certifications in the area of diabetic health managementCPR/First Aid CertificationRequired Work Experience:* Minimum of two (2) years of experience in a related clinical or hospital environmentPreferred Qualifications: One (1) year of working experience in a medical or physician office setting. Knowledge of The Joint Commission Regulations.
