Your recruiter will provide the most up-to-date details during the hiring process._**How you'll make an impact in this role**+ Lead personalized care plans to ensure patients meet their recovery goals through active monitoring and adjustment.+ Advocate for patient safety by communicating real-time health updates to the medical team for informed decision-making.+ Coordinate holistic healing by acting as the primary link between all clinical specialties and the patient.+ Champion the transition home by collaborating with the healthcare team on seamless, safe discharge planning.**What minimum requirements you'll need**Licensure / Certification / Registration:+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
