DIRECTOR OF HEALTH INFORMATION MANAGEMENTCascades HealthcarePosition Summary:The Director of Health Information Management (HIM) is responsible for planning, organizing, and directing all health information management functions across the company. This role ensures the accuracy, integrity, security, and accessibility of resident health records in compliance with federal and state regulations governing skilled nursing facilities, including CMS Conditions of Participation, HIPAA, and applicable state licensure requirements.Essential Duties and Responsibilities:Health Records Management• Oversee the maintenance, organization, and retrieval of all resident medical records in both electronic and paper formats• Ensure timely completion of medical records, including physician signatures, discharge summaries, and required documentation• Manage medical record audits to assess documentation accuracy, completeness, and compliance• Direct the release of information process in accordance with HIPAA and facility policyCoding & Reimbursement• Supervise or perform ICD-10-CM/PCS coding for MDS-related diagnoses and other clinical documentation• Collaborate with MDS Coordinator and billing team to support accurate RAI/MDS completion and optimize appropriate reimbursement under PDPM• Monitor coding accuracy and provide ongoing education to clinical and coding staffCompliance & Regulatory• Ensure compliance with federal and state regulations, including CMS, state health department, and HIPAA requirements specific to long-term care• Serve as the facility's HIPAA Privacy Officer or support Privacy Officer functions• Prepare for and participate in state surveys and CMS audits related to medical records• Develop, implement, and maintain HIM policies and proceduresData Integrity & Analytics• Monitor and report on key HIM metrics including record completion rates, coding accuracy, and audit findings• Collaborate with the interdisciplinary team to support quality assurance and performance improvement (QAPI) initiatives• Maintain data integrity within the Electronic Health Record (EHR) systemLeadership & Education• Recruit, train, supervise, and evaluate HIM department staff• Provide ongoing education to clinical staff on documentation requirements, chart completion, and regulatory standards• Act as a resource to administration, nursing, and therapy regarding medical record and documentation issuesMinimum Qualifications:• Bachelor's degree in Health Information Management or related field required; Master's degree preferred• Registered Health Information Administrator (RHIA) required; RHIT considered with substantial experience• Minimum 3-5 years of HIM experience, with at least 2 years in a long-term care or skilled nursing setting strongly preferred• Working knowledge of ICD-10-CM coding, MDS/RAI process, and PDPM• Familiarity with EHR systems used in SNF settings (e.g., PointClickCare, MatrixCare)• Strong understanding of HIPAA, CMS Conditions of Participation, and state SNF regulationsPreferred Qualifications:• Experience serving as HIPAA Privacy Officer• Knowledge of RAC, MAC, and ADR audit processes• Prior experience managing or mentoring HIM staff• Certified Coding Specialist (CCS) or Certified in Healthcare Privacy and Security (CHPS) credentialCore Competencies:• Attention to detail and commitment to data accuracy• Strong organizational and time management skills• Effective communication with clinical, administrative, and compliance teams• Ability to interpret and apply regulatory requirements• Leadership and staff development capabilitiesWorking Conditions:This is an office-based role and will include periodic on-site visits to the facilities.
